- Singapore Citizen or Permanent Resident
- Formal diagnosis of dementia by a doctor
- Clinical stable and free from active infectious disease
Two Routes to Admission
Subsidised Residential Care
Families who qualify for government subsidy may apply through the Agency for Integrated Care (AIC). Subsidy levels are determined by means testing and vary according to household income.
The process is as follows:
- Approach the Medical Social Worker (MSW) at a restructured hospital or polyclinic
- Declare household income for means testing to determine subsidy eligibility
- The MSW will submit a referral to AIC
- AIC will assess the application and match your loved one to a suitable nursing home based on care needs, availability, and preferences
- Once matched to Apex Harmony Lodge, our team will contact you to arrange a pre-admission assessment and plan the transition together
Private Residential Care
