Founded in 1999, Apex Harmony Lodge is a 210-bed nursing home dedicated to delivering specialized, person-centred dementia care. The Lodge is committed to advancing excellence in dementia care through clinical innovation, interdisciplinary collaboration, workforce capability development, and evidence-based practice. We strive to create a therapeutic environment that promotes dignity, independence, functional ability, and quality of life for persons living with dementia and complex geriatric conditions. 

Position Summary 

The Physiotherapist is responsible for the assessment, planning, implementation, and evaluation of individualized rehabilitation programs for residents at Apex Harmony Lodge. The incumbent plays a key role in enhancing residents’ mobility, functional independence, physical wellness, fall prevention, and overall quality of life through evidence-based physiotherapy interventions tailored to the needs of older adults living with dementia. 

The Physiotherapist will work collaboratively within a multidisciplinary care team to provide holistic rehabilitative care while ensuring compliance with professional standards, regulatory requirements, and organizational policies. The role also includes staff training, mentorship of therapy assistants, participation in quality improvement initiatives, and contribution to the Lodge’s strategic rehabilitation and dementia care programs. 

Key Responsibilities 

  1. Clinical Assessment & Rehabilitation Planning
  • Conduct comprehensive physiotherapy assessments of residents’ physical, functional, cognitive, behavioural, psychosocial, and mobility needs.  
  • Develop individualized rehabilitation care plans with measurable goals aligned to residents’ functional abilities and person-centred care objectives.  
  • Perform risk assessments related to falls, mobility decline, deconditioning, pressure injuries, pain, and transfer safety.  
  • Review residents’ rehabilitation outcomes regularly and modify treatment plans based on progress and clinical needs.  
  1. Therapeutic Intervention & Clinical Care
  • Deliver evidence-based physiotherapy interventions to improve mobility, balance, strength, endurance, posture, gait, and functional independence.  
  • Implement therapeutic exercise programs, mobility training, transfer training, pain management strategies, and falls prevention interventions.  
  • Utilize appropriate therapeutic modalities and rehabilitation techniques in accordance with professional standards and residents’ clinical conditions.  
  • Support restorative care approaches that maximize residents’ participation in activities of daily living and maintain functional capacity.  
  • Recommend and advise on mobility aids, positioning strategies, seating systems, and environmental modifications to support safety and independence.  
  1. Dementia-Specific Rehabilitation Care
  • Adapt rehabilitation approaches to meet the behavioural, cognitive, communication, and psychosocial needs of persons living with dementia.  
  • Apply dementia-friendly and trauma-informed care principles during therapy interventions.  
  • Collaborate with nursing and care teams to manage responsive behaviours impacting rehabilitation participation and mobility.  
  • Promote meaningful engagement and therapeutic activities that support residents’ emotional and psychosocial well-being.  
  1. Multidisciplinary Collaboration
  • Work closely with doctors, nurses, occupational therapists, psychologists, therapy assistants, social workers, and other allied health professionals to coordinate holistic resident care.  
  • Participate actively in multidisciplinary case discussions, care planning meetings, ward rounds, and family conferences.  
  • Provide clinical recommendations to support safe discharge planning, mobility management, and long-term care goals where applicable.  
  1. Staff Training, Education & Capability Development
  • Provide education and coaching to residents, caregivers, and staff on exercise programs, mobility techniques, falls prevention, and safe handling practices.  
  • Lead orientation, competency training, and clinical supervision for therapy assistants and rehabilitation support staff.  
  • Serve as a clinical mentor and preceptor to junior staff and students during clinical attachments.  
  • Support departmental capability-building initiatives, in-service education, and continuous professional development programs.  
  1. Quality Improvement & Program Development
  • Participate in quality assurance, clinical audits, research, and evidence-based practice initiatives to enhance rehabilitation outcomes.  
  • Contribute to the development and review of rehabilitation protocols, workflows, and dementia care programs.  
  • Support innovation and implementation of rehabilitation initiatives aligned with organizational strategic goals and regulatory standards.  
  • Monitor service delivery indicators and recommend improvements to enhance clinical effectiveness and operational efficiency.  
  1. Documentation & Administrative Responsibilities
  • Maintain accurate, timely, and comprehensive clinical documentation in accordance with professional, legal, and organizational requirements.  
  • Ensure compliance with applicable healthcare regulations, including the Healthcare Services Act (HCSA), Allied Health Professions Council (AHPC) standards, and institutional policies.  
  • Prepare clinical reports, rehabilitation statistics, outcome measures, and departmental records as required.  
  • Support operational and administrative functions within the Rehabilitation Department when assigned.

Job Requirements 

  • Bachelor’s Degree in Physiotherapy from a recognized institution.  
  • Fully registered with the Allied Health Professions Council (AHPC), Singapore, with a valid Practising Certificate.  
  • Minimum 3–5 years of relevant clinical experience in physiotherapy, preferably in geriatric, community hospital, rehabilitation, or long-term care settings.  
  • Experience in eldercare and dementia care will be highly advantageous.  
  • Demonstrated competency in falls prevention, mobility rehabilitation, restorative care, and functional maintenance programs.  
  • Experience in staff training, supervision, or capability development is preferred.  
  • Strong communication, interpersonal, and documentation skills.  
  • Proficient in multidisciplinary collaboration and person-centred care delivery.  

Preferred Attributes 

  • Passion for dementia care and rehabilitation of older persons.  
  • Familiarity with InterRAI assessments and long-term care regulatory frameworks.  
  • Experience in quality improvement or program development initiatives.  
  • Ability to communicate effectively with residents, families, and caregivers from diverse backgrounds.
